In 2019, HAI began working with the Mother Support Groups (MSG) again with the aim of strengthening their implementation. HAI works with the health providers that support the MSGs in 10 suco in Aileu and 10 suco in Ainaro by supporting them to improve their facilitation and work planning skills, as well as refreshing their nutrition and general family health knowledge. The goal of the MSGs is to promote key home practices focused on nutrition with the aim of improving maternal and child health outcomes. The groups are composed of community volunteers, both men and women. Following Estrela+’ success as a Sub-Sub-Recipient under FTH (Fundasaun Timor Hari’i) in Round 10 Phase 1 of the Global Fund to end AIDS, Malaria and Tubeculosis (Global Fund) Grant to Timor-Leste in 2012-2013, Estrela+ became a Sub-Recipient under Phase 2 for the period 2014-2016. Under the Program ‘Strengthening PLHIV networks in Timor-Leste’, Estrela+ will undertake an extensive schedule of activities over the next three years that are aimed at strengthening the PLHIV network in Timor-Leste. Since 2006 Fundasaun Alola has extended its MCH program to include maternity packs, women's health and newborn care. NBFA was handed over to the Ministry of Health (MoH) Nutrition Department in 2009. To facilitate the handover process, Fundasaun Alola has seconded a senior staff member for two days to work with the MoH. The staff member will assist in coordinating and organizing the implementation of Baby Friendly Hospital Initiative (BFHI), providing breastfeeding training for health workers and advocating for a breastfeeding policy and an international code for breastmilk substitutes. Infant and young child feeding is a global strategy that was adopted by Timor Leste in 2006. It is now a national program at MoH under the Nutrition Department and Fundasaun Alola is responsible for running it across the country. Fundasaun Alola is involved in the training of health care workers in infant and young child feeding and Alola also established Maternal and Child Health Community-Based Groups, specifically Mothers Support Groups (MSG) and Suku Hadomi Inan no Oan (Village Loves it Mother and Children) (SHIO). These groups are the target to disseminate the information to encourage and support mothers to initiate breastfeeding immediately after delivery, to breastfeed exclusively for the first six months of life and to continue breastfeeding for at least two years along with introduction of appropriate complementary feeding.

- Putting health in the hands of mothers - Increase in births with skilled birth attendants - Tau saúde iha inan sira nia liman - Incrés partus ne’ebé aumenta ho parteira Liga Inan is using mobile phones to connect expectant mothers with health providers in Timor-Leste to improve the likelihood of a healthy pregnancy, birth and post natal period. Officially launched in March 2013, Liga Inan is implemented by Health Alliance International (HAI) and Catalpa International, in partnership with the Ministry of Health. The pilot of the Liga Inan program was funded by USAID’s Child Survival and Health Grants Program, with the Australian Government now funding the further development and expansion of Liga Inan across Timor-Leste. Liga Inan cuts across geographic barriers to directly connect women to better health care in a country with high maternal and neonatal mortality. Health providers use a mobile phone to register expectant mothers, track their progress, know when they go into labour and make arrangements for attending the delivery. Mothers continue to receive support through the first six weeks of the newborn's life. Liga Inan also sends automated promotional health information via SMS to all registered pregnant mothers throughout their pregnancy and for six months after delivery to help them make better decisions about their pregnancy and link them with regular pre and post-natal care. HAI and other health partners have supported the Timor-Leste National Institute for Health (INS) to conduct national in-service and refresher training across the maternal care continuum. With USAID and Australian Government funding, HAI’s staff of midwife trainers have supported national training for municipality midwives in essential newborn care, safe and clean delivery and family planning, and also supported refresher training for basic emergency obstetric care. After large national training efforts, a coordinated system of follow-up-after-training (FUAT) and supportive supervision for quality improvement is needed. To address the ‘FUAT gap,’ HAI in partnership with INS developed and implemented an innovative approach we call Learning Labs. These are a cost-effective and practical way of developing health provider core competencies, improving provider-client communication, and strengthening provider motivation. Facilitated monthly for six months in the midwives’ own health facilities, adult learning approaches are applied through use of case-studies, practice simulations, and role plays. The Learning Lab is underpinned by the principles of adult learning, engaging with the trainee through a respectful, professional, and caring attitude to enhance a supportive learning environment. Addressing issues of domestic violence and the needs of women with disability have been incorporated into the Learning Lab model. An evaluation of the Essential Newborn Care Learning Lab indicated that all the providers who participated in Learning Labs achieved competency or proficiency after six months, compared to none in the control site. Following this successful evaluation, a Learning Lab was developed for Safe and Clean Delivery, where the evaluation similarly showed that midwives who participated in Learning Labs achieved competency or proficiency of skills as compared to sub-optimal competency levels pre-intervention. A Learning Lab on Antenatal Care (ANC) has recently been added.
