- Putting health in the hands of mothers - Increase in births with skilled birth attendants - Tau saúde iha inan sira nia liman - Incrés partus ne’ebé aumenta ho parteira Liga Inan is using mobile phones to connect expectant mothers with health providers in Timor-Leste to improve the likelihood of a healthy pregnancy, birth and post natal period. Officially launched in March 2013, Liga Inan is implemented by Health Alliance International (HAI) and Catalpa International, in partnership with the Ministry of Health. The pilot of the Liga Inan program was funded by USAID’s Child Survival and Health Grants Program, with the Australian Government now funding the further development and expansion of Liga Inan across Timor-Leste. Liga Inan cuts across geographic barriers to directly connect women to better health care in a country with high maternal and neonatal mortality. Health providers use a mobile phone to register expectant mothers, track their progress, know when they go into labour and make arrangements for attending the delivery. Mothers continue to receive support through the first six weeks of the newborn's life. Liga Inan also sends automated promotional health information via SMS to all registered pregnant mothers throughout their pregnancy and for six months after delivery to help them make better decisions about their pregnancy and link them with regular pre and post-natal care. The overarching goal of the Learning Lab program is to reduce maternal and infant mortality. HAI and other health partners have supported the Timor-Leste National Institute for Health (INS) to conduct national in-service and refresher training across the maternal care continuum. With USAID and Australian Government funding, HAI’s staff of midwife trainers have supported national training for municipality midwives in essential newborn care, safe and clean delivery and family planning, and also supported refresher training for basic emergency obstetric care. After large national training efforts, a coordinated system of follow-up-after-training (FUAT) and supportive supervision for quality improvement is needed. To address the ‘FUAT gap,’ HAI in partnership with INS developed and implemented an innovative approach we call Learning Labs. These are a cost-effective and practical way of developing health provider core competencies, improving provider-client communication, and strengthening provider motivation. Facilitated monthly for six months in the midwives’ own health facilities, adult learning approaches are applied through use of case-studies, practice simulations, and role plays. The Learning Lab is underpinned by the principles of adult learning, engaging with the trainee through a respectful, professional, and caring attitude to enhance a supportive learning environment. Addressing issues of domestic violence and the needs of women with disability have been incorporated into the Learning Lab model. An evaluation of the Essential Newborn Care Learning Lab indicated that all the providers who participated in Learning Labs achieved competency or proficiency after six months, compared to none in the control site. Following this successful evaluation, a Learning Lab was developed for Safe and Clean Delivery, where the evaluation similarly showed that midwives who participated in Learning Labs achieved competency or proficiency of skills as compared to sub-optimal competency levels pre-intervention. A Learning Lab on Antenatal Care (ANC) has recently been added. Timor-Leste has the highest rate of child malnutrition in Asia with over half of children under five stunted (low height for age). Research shows that rates of exclusive breastfeeding vary widely across the country (28-75%) and there is a lack of dietary diversity with only 18% of children under two consuming a minimum acceptable diet. HAI was awarded a grant by UNICEF to establish and mentor Mother Support Groups (MSGs) in two municipalities in Timor-Leste. The goal of the MSGs is to promote key home practices focused on nutrition with the aim to improve maternal and child health outcomes. The groups are composed of community volunteers, both men and women, and are implemented in every village in the municipality. HAI works closely with the Ministry of Health and the municipality health team to establish and run the groups. It is intended that MSGs will be mainstreamed into the duties of village-based doctors and will strengthen the link between health providers and the communities they serve. • Village based doctors • Mothers and fathers of children under five • Children under five

This is a long-term goal that contributes directly to the overall strategic and development goals of Timor-Leste as expressed in the Timor-Leste Strategic Development Plan (2011-13) and the clear intent of the ‘Zero Hunger Challenge’. Success of project development objective is dependent on the success of other food security related projects, and impacts from this project should be measured beyond the timeline of project activities (2025). Indicators of success are: • Increased fish consumption by children and pregnant women • Reduced incidence of stunted growth (malnutrition) in children under five The immediate objective is to ‘increase the consumption of marine fish products among rural households in Timor-Leste’. This is in-line with Government priorities to improve the food and nutrition security of the people of Timor-Leste. Indicators by 2020: • Increased availability of affordable marine products in rural areas • Increased marine fish portion in average food basket and daily household diet Affordability of products can be assessed through the combined use of market and household surveys. Consumption, with an emphasis on pregnant women and children under 5, will be measured through household surveys. Carried out activities including: (i) to strengthen capacity of the Ministry of Agriculture and Fisheries (MAF) suco extension officers (SOEs) to provide specialized technical support to farmers in their geographic target areas; (ii) to increase capacity of 4,500 farmers in farming and storage techniques for improved and diversified production; (iii) to facilitate market linkages for target households to increase income from sales from crops; and (iv) to improve capacity of 4,500 households with household resource management skills. The final evaluation of the programme was carried out in August 2013, and the findings have been incorporated into this report. The final evaluation found that the project has been successful in meeting most of its targets, and for many, exceeded them. It found that 73% of households in target areas reported a reduction in food shortage with the number of households not able to meet food needs reduced significantly from 80% to 15% and the average number of food scarce months reduced from 3.9 to 1.2 in the last year. 88% of households reported increased income. In addition there was also improvement in the Household Dietary Diversification Score (HDDS) from 6.3 to 7.9 (of 12 food groups).

KAP was designed to gain a broader understanding of the knowledge, attitudes and practices associated with nutrition related practices among the households in the TOMAK target implementation areas This assessment was largely built upon existing secondary data. The assessment comprised of combinations of focus group discussions (FGDs), key informant interviews (KIIs) and surveys aimed at gaining an overall understanding of target households and their members’ nutrition promoting behaviours. The findings will be used by TOMAK to develop a theory of change to trigger household demand for year-round production and utilization of diverse and sufficient foods. The Objective of this survey was to gather up-to-date and gender-specific information about the knowledge, attitudes and practices (KAPs) related to nutrition of a cross section of the proposed TOMAK’s target population. Establishing a foundation of food security and good nutrition requires stimulating household demand for year-round production and utilisation of nutritionally diverse and sufficient foods. To effectively stimulate this demand, TOMAK needs to understand the demographic profile and context in which choices are made around nutrition of its target beneficiaries. With this information, TOMAK will be able to design and invest in both nutrition-sensitive agriculture (NSA) and social behaviour change communication (SBCC) interventions. Such an understanding requires data on the target beneficiaries: (1) present food consumption patterns; (2) knowledge, attitudes and practices (KAP) related to food consumption; and (3) how these households commonly access and use information. The specific areas of enquiry were related to the following behaviours: Infant and Young Child Feeding (IYCF) practices; Feeding practices during pregnancy and lactation; Critical handwashing practices; Income allocation for household nutrition; and Relevant roles around decision-making within the household.
